Re : patient Saif Salem Mubarak Al- Gafli* feb.14,1974 , no:302575CPC
Dear Sirs:
I wish to report about the above named, on whom I carried out a follow-up examination on Feb.9,2004.
As you know, Mr. Al Gafli was operated on Jan.20 ,2004 . the following operation was performed: Decompression of the cervical cord and the roots C5 on both sides by removal of the anterior and posterior spondylophytes, the thickened posterior intervertebral ligament and the medial disk prolapse, foraminotomies on both sides, open correction of position by 2 degrees, with installment of a supporting device with a disk prothesis at cervical vertebral bodies 4/5 in microsurgical technique due to cervical myelopathy.
The patient reported that he feels well . he has tingling paraesthesias on the hands and finger pulps. Walking has improved .
Physical examination revealed a slight improvement of the paraspasticity in the legs. Mr. Al Gafli stated a hypaesthesia on the finger pulps and the palms of both hands, more so in the palm of the right hand.
In view of the significant improvement of the clinical symptoms, I recommend continuation of physical therapy, with step-by-step mobilization. The patient will still be unfit for work until the end of April 2004 . he can then try to start working again in his profession.
I also recommend a follow-up examination in 1 year, including radiologic views.
Best regards
